Intellectual/Developmental Disability Services

Developmental Day Support

There are three Creative Innovation centers located in Halifax, Mecklenburg and Brunswick counties, which offer Developmental Day Support program services for adults with intellectual/developmental disabilities.

  • Persons with intellectual/developmental disability 22 years of age and older (or age 18 with parental consent) receive training in independent living skills, basic educational training, self-help, and community engagement.
  • Tailored service plans are specifically developed for each individual using a person-centered planning approach, and the particular needs and preferences of each individual are assessed and continually monitored.
  • Support Coordinators facilitate an individual's referral to and participation in programs offered through the Creative Innovation Centers.

Support Coordination

Individuals may be assigned a support coordinator, who monitors services needed and develops an individual service plan. Developmental disability support coordinators provide ongoing assessments of individuals; refer and link them to needed services; and oversee the continuity of services provided for each individual. Support coordinators also provide discharge planning and referral coordination for aftercare and follow-up services.

Residential Services

  1. Group Homes
    SSCSB group homes provide residential services to adults with an intellectual/developmental disability. Referrals are processed through support coordinators. Individualized service plans are developed for each resident with specific behavioral objectives. Close follow-up is maintained with families and referring agencies. Individuals receive day support program services at the applicable SSCSB Creative Innovation Center, or another appropriate service location of their choice.

  2. Semi-Independent Living (One-bedroom Apartments)
    Semi-independent living provides a progressive step in the continuum of care for individuals with intellectual/developmental disability. Residents must have demonstrated the ability to perform basic activities of daily living (bathing, personal hygiene, grooming) with minimal to no assistance; must have some knowledge of money and money management; must have basic cooking and housekeeping skills; and must have knowledge of health and safety issues (including medication compliance). Candidates for admission will function in the mild to moderate (high) range of intellectual/developmental disability; they can be non-ambulatory but must demonstrate the ability to transfer independently. Skill demonstration will be required prior to admission. 

  3. Intermediate Care Facilities
    Hope House is an eight (8) bed Intermediate Care Facility for people with severe to profound levels of intellectual disabilities (ICF-ID/DD), and/or individuals with an intellectual/developmental disability having a medical diagnosis which requires specialized care. Located in La Crosse, Mecklenburg county, the ICF-ID/DD provides intensive, highly supervised residential and specialized care in maintaining health status and supporting adaptive behavior to enhance an individual's level of functioning. Other areas of assistance include nursing and supportive services (i.e. occupational, physical and speech therapies, behavioral training, dietetic consultation, psychological and support coordination services, as well as intensive supervision 24/7. In addition, local contacts will be made in the surrounding communities to facilitate the implementation of services identified in the service plan. Brandon Home, a four (4) bed ICF-ID/DD located in South Boston, Halifax County, offers the same intensive, highly supervised residential support services.
